Out of Hours Appointments

Did you know you can now book appointments with a GP weekday evenings from 6:30pm to 8:00pm, Saturday's and Sunday's and also weekend evenings at one of the Rotherham Hubs. 

If you wish to be seen at any of the Hubs (Dinnngton, Broom Lane, Magna Surgery) please contact the surgery for a member of staff to book you into one of the available slots. When booking the appointment a member of staff will ask you the following questions, 

  • Reason for appointment
  • Consent for 3rd party access to your clinical records (without this you will not be able to attend the HUBS).
  • Do you require a chaperone.
  • Is this an ongoing problem. (Please note the appointment won't be booked if it is an ongoing problem you are seeing the GP about). 
  • Up to date contact telephone number. 

Travel Information

If you require any vaccinations relating to foreign travel you need to fill a travel vaccination form below or call into surgery to complete.  The nurse will then look to see what vaccination are required for you. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.  You will need to contact the surgery after a week of handing the forms in and an appointment will be made for you.

It is important to make this initial appointment as early as possible  - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations.  These vaccines have to be ordered as they are not a stock vaccine.  Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.

To help us offer the appropriate advice, please call in and collect the surgery travel health questionnaire, hand it in and then contact the surgery after 3 working days when we will let you know if you require any vaccinations and the schedule to which you should work
