Are you self-Isolating
For information regarding self-isolating and your rights please refer to the government websites, www.gov.uk The practice is not able to issue sick notes during this period but you can complete an online self isolation form from www.gov.uk
Appointments booked with a GP will be converted to a telephone consultation, the doctor will call you on the number we hold on your record, if this is incorrect please contact the surgery.
All nurse clinics (including Phlebotomy) have also been cancelled until further notice. We will be contacting you at a later date with a new appointment. At present we will still be running the baby immunisation clinic on Wednesday.

All repeat prescription requests should now be requested over the telephone on 01709 533050, we appreciate that this line will be busy so ask that you bear with us and try at various times though out the day, it is open from 10 am until 4 pm. Could we also ask that you contact a pharmacy of your choice and ask them to nominate them to allow us to send your prescription electronically. We will not be giving out prescription at the practice anything that needs to be printed by the surgery will be sent to a pharmacy of your choice.

When the Surgery is closed, either at the weekends or for Public Bank Holidays and you require urgent medical Out of Hours assistance, which cannot wait until the surgery re-opens, please call 111 (this is a free phone number from both landlines and mobiles)

You can also seek help from your local Pharmacy for non-life threatening
NOTICE: Patients are able to have access to Coded Data held on their medical records but not access to Full Medical Records.
On line access held by parents on behalf of their children ceases automatically when the child reaches the age of 13 and must be re-applied for by the child.
