Please note that when you ring the practice for an appointment the reception staff will ask you for a brief summary of your problem in order to direct you to the most appropriate service.


All  routine until further appointments, will be telephone consultation.  Please ensure when booking an appointment you give the surgery an up to date contact number.  Should you need a face to face appointment the doctor will book you in following your telephone consultation.

You can now book face to face appointments with the Plebotomist and Practice Nurse.

Out of Hours Appointments

Did you know you can now book appointments with a GP weekday evenings from 6:30pm to 8:00pm, Saturday's and Sunday's and also weekend evenings at one of the Rotherham Hubs. 

If you wish to be seen at any of the Hubs (Dinnngton, Broom Lane, Magna Surgery) please contact the surgery for a member of staff to book you into one of the available slots. When booking the appointment a member of staff will ask you the following questions, 

  • Reason for appointment
  • Consent for 3rd party access to your clinical records (without this you will not be able to attend the HUBS).
  • Do you require a chaperone.
  • Is this an ongoing problem. (Please note the appointment won't be booked if it is an ongoing problem you are seeing the GP about). 
  • Up to date contact telephone number. 

New Patient Registration

We currently have an open list and welcome registrations from people living in or  moving to the practice area  

If you move out of the area we regret you will need to register with another practice.  You are registered with the Practice rather than an individual GP and may see any of the doctors.  You are able to express a preference of  GP and whilst the practice will try to comply it may not be always possible in which case we will tell you why.  

To register please call in and collect a registration pack from reception or download the forms below.

Please bring proof of your identification (photographic, i.e passport) and confirmation of address, when coming to register at the practice.  This is to help the staff but is not a barrier for you registering at the practice

You will be given a health form to complete and return as soon as possible.  If you are on repeat medication then an appointment will be booked to see either a GP or our in house pharmacist.  

It is important that you keep us informed of any changes to your circumstances i.e. address, telephone number in order for us to be able to contact you should the need arise.

Please be aware that we do not register patient who live outside of our practice area or allow patients to stay on our register if they move out of area.
